Elementary Sports Waivers Deadline

Submit the required waivers through DragonFly. The Elementary Athletics department is hosting a pop clinic this Saturday (10/25/25). See the instructions below to sign up for DIA's elementary program:

1. Visit dragonflymax.com and click the **'Log In/Sign Up'** button.

   • *Important:* Avoid using your child’s name or contact details for this step—you’ll be able to add their information later.

2. Verify your account using the verification ID sent to your email.

3. Click **'Connect to your school'**, choose **'Parent'** as your role, and search for your child’s school by entering **NMCU5W**.

4. After locating your child’s school, click **'Join'** to request access. The school administrator will review and approve your request.

5. Once approved, click **'Set up your children'** and follow the steps to add your child(ren) and complete their participation forms.

We're hosting a pop up clinic October 25th. Register before 9:30am October 25th if you don't already have an up-to-date physical evaluation.

• If you prefer to use your own care provider, please print the evaluation form below for them to complete.
